Visa application form. Fully completed and signed application form.
Original passport. The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.
Passport-type photograph. A digital passport-type photograph taken within the last 6 months. This may be taken using a cell phone against a white wall provided if it meets the standards of a passport-type photograph.
Health insurance. Proof of international health insurance with a minimum coverage of EUR 30,000, specifically covering travel to Ukraine is required. The policy should cover all expenses for repatriation, emergency medical aid, and hospital treatment for the period of stay. Applicants can get a free quote and purchase valid travel insurance from our affiliate.
Bank statement. A copy of the applicant's most recent monthly bank statement showing proof of sufficient funds. The statement must clearly show the applicant's name as the account holder, the balances of the accounts, and the date of the statement.
Notarized birth certificate. An original legalized birth certificate, along with a notarized translation in Ukranian. Notarization instructions can be found here.
Transcripts. Legalized originals of transcripts and any degrees/certificates, along with notarized translations in Ukranian. Notarization instructions can be found here.
Original letter of invitation. An original letter of invitation to study from the educational institution in Ukraine.
Medical Certificate. A legalized original medical certificate, certified by the official health authority in USA, along with a notarized translation in Ukranian. Notarization instructions can be found here.
Visa application form. Fully completed and signed application form.
Original passport. The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.

Itinerary. A copy of round trip tickets or detailed itinerary for the duration of the trip. The applicant's name must be on itinerary.

Employment letter. Employment Authorization Document (Dozvil Na Pratsevlashtuvannia) in the name of the applicant issued by the Ukrainian State Center of Employment and obtained for him/her in advance by the employer in Ukraine.
Business registration. A copy of the host organization's Certificate of Incorporation or business registration in Ukraine.

Ukraine Visa Policy
According to the visa policy of the Ukrainian government, visitors must get a visa from a consulate or online if they wish to visit the country unless they are from a visa-exempt country. This policy segments foreign nationals into different categories with varying benefits and application requirements. For instance, holders of passports and national ID cards, in some cases, are allowed to enter and stay in Ukraine without visas for different periods. Others are required to provide additional documents and proof of funds before they can be allowed into the country with or without visas based on their nationality.
There is also a visa waiver program for citizens of Mongolia but they are yet to be ratified.
Diplomatic and Service passport holders from the following countries do not need visas for Ukraine for short visits.
Visitors that qualify for visa free entry are only allowed into the country for the purpose of tourism, business or if they are international travelers passing through Ukraine for another country.
Ukraine Visa On Arrival
The visa on arrival has yet to be in place, as it was terminated in January 2019. Such visas were previously issued at the Kyiv International Airport, Odesa International Airport, and Kyive Boryspil Airport for visits of up to 15 days, but it has been replaced with an electronic visa.
Ukraine Electronic Visa
The electronic visa of Ukraine was launched on the 4th of April 2018 for business, transit, and tourism purposes for 30 days per visit. This visa program was further expanded more than a year later to cover medical treatments, education, science, culture, and sports for foreign correspondents working for foreign media representatives. E visas are issued as single-entry travel documents and take three working days to process.
To qualify for this visa you must be from an eligible country. You must also complete an online application form and upload soft copies of your passport and any other visa required. Once the visa is approved, it will be forwarded to your email. Endeavor to print it out and travel with a copy to Ukraine as the Immigration officers in duty will request for it. After close examination, they will stamp your passport any let you in.
Note: The e-Visa program has been suspended for now due to the Russo – Ukrainian War.
Key Information About Ukraine e Visa
Here are some things to bear in mind when applying for a Ukrainian visa
Please obey the rules: It is very important to obey the local rules and regulations during your time in the country. Only engage in activities permitted by your visa, and do not stay beyond the days allowed. If your visa allows you to stay for 30 days, do not stay for 31 days, as the extra one day is a violation.
Use Approved Routes: Always enter Ukraine through approved airports and seaports. If you decide to use a different route, you may be prevented from entry.
Use The Same Passport: Your e visa is linked to your passport so make sure you travel with the same passport you used for the application
If you bring a different passport you will be denied entry because it will be assumed you don’t have a visa.
It Is Not For Residency: Bear in mind that the online visa is not a long stay visa for residency but for short visits. Studying or working in Ukraine with an e visa is prohibited because it is only designed for tourism, transit and business. If you have any reason to stay for more than 30 days you must apply for a consular visa instead.
Non Transferrable: This online visa is not transferable so two people cannot use the same visa. There isn’t a group e visa for tourism but each person must apply for their own visa in advance of their trip. Minors also need their own online visas which their parents or guardians can apply for. Parents must bring civil status certificates along like a birth certificate to prove parental relationship while guardians must bring a notarized parental consent letter proving that the child’s parents approve of the trip.
Extension: If you must apply for an extension of your visa you should do so before your current visa expires. Persons who apply for extensions after their visa expires may not have it granted because they would be living in the country illegally even if it is for a few days. Consider applying for extension at least a week before your visa expires. This will give the authorities ample time to process your application.
Letters Should Be Signed: Whether it is a letter of invitation from your host, a cover letter explaining your purpose, or a letter from your employer, all letters must be signed and dated. Furthermore, they should contain relevant information about your trip to validate whatever information you give the authorities in case you are asked.
