Notice of the Elimination of Visa Page Inserts for U.S. Passports
The U.S. Department of State will no longer add visa pages into U.S. passports beginning January 1, 2016. Previously, U.S. passport holders had the option to pay for the insertion of additional 24-page visa inserts when valid passports lacked adequate space for entry or exit visa stamps. The decision to discontinue this service was made to enhance the security of the passport and to abide by international passport standards.
To mitigate the impact on frequent travelers, the Department began issuing 52-page passports to all applicants outside the United States starting October 1, 2014 for no additional cost. Applicants within the United States may choose a 28-page or 52-page book.
Requests for additional 24-page visa inserts will only be accepted until December 31, 2015 by the Department of state. In order to meet this deadline please be aware that the last date to place an order for additional pages will be December 28, 2015. - BApplication for additional visa pages: DS-4085 online
- Click on "Submit" under "Add Visa Pages" column, complete all steps and print out the application. Be sure to sign and date your application. The DS-4085 form must be completed online, applications without a barcode will be delayed.
- CLetter of Authorization
- Please download, print and sign 2 copies of the Letter of Authorization.
- DValid passport
- Please make sure that the expiration date is more than 1 year away. If your passport expires within a year apply for passport renewal.
- ETravel itinerary
- - Copy of a plane ticket to a foreign country, or
- Copy of a travel itinerary with the applicant's name showing dates and flight info, or if neither is available and traveling for business
- A letter from the employer.
For 5-7 business day passport service only travel dates must be within 14 days.
